The process always begins with a thorough Assessment and Client Evaluation. We will ask you and your loved one(s) several questions, listen carefully, and determine what options you will have.  

The Care Plan directs the care to be provided.  It is a document that is constantly evolving and updated as your loved one's needs change.  

All of the service providers need to be coordinated and managed.  There will be many phone calls that we will handle and share the outcomes with the family when the issue has been resolved.  

We have built a network of trusted professionals  who have the  expertise to implement the care plan and report appropriate changes  in condition in a timely manner so adjustments can be made to the care plan.

Having professional expertise attend physician appointments is critical to understanding the details of the appointment to share with family  members.

We have extensive knowledge of the requirements for Medicare, Medicaid, Hospice and Long Term Care Insurance.  We will make sure your claims are processed correctly and that all of the eligibility requirements are met.

 The process always begins with a thorough Assessment and Client Evaluation. We will ask you and your loved one(s) several questions, listen carefully, and determine what options you will have for insuring the highest level of safety and happiness for your senior loved one. The Assessment is generally done in person where the senior lives but can also be done in a hospital or rehab center. 

Following the Assessment and Evaluation, a Care Plan will be developed that will guide the care options available. We will research available resources (including Medicare Skilled Services, in Home Care, assisted living, etc.) and will educate the family on what the services entail, what payment options are available, various price points for the different options and set realistic expectations. There are often many possible solutions for any situation. The Care Plan will be written and reviewed with the family to ensure all family members are informed and in agreement. 

We will actively manage the Care Plan and stay involved with adjusting the Care Plan as your loved one improves or possibly declines. Our goal is always safety and happiness. Sometimes the level of care can be decreased as your loved one’s situation improves. In some situations, the level of care required to keep your senior safe and have their needs appropriately met will mean the services may need to be increased. In either situation, the family will be kept fully informed and knowledgeable of whatever changes are happening and what the financial implications are, if any.
